The QSFP-100G-SR4-S is Cisco's standard multimode QSFP28 100G module for data center spine-leaf architectures and Nexus 9300/9500 and UCS interconnections. It supports distances of 70m over OM3 and 100m over OM4. It uses 4 parallel 25G lanes over MPO-12 fiber. Complete data sheet.
